Change in Transportation / Early Dismissal Form

Parent/guardian can complete this form to request a change in dismissal routine OR early pick up for your child.  All online requests should be made by 12:00 p.m.  For any changes or emergencies after 12:00 pm, please contact the office at 703-764-5600.
  • Form response will be forwarded to the classroom teacher indicated.  If you have multiple Canterbury Woods students for whom this change applies, you must complete this form for each child. 
  • When a student is going home with another student, it is expected that parents have coordinated plans beforehand.  Each family must submit the online form by 12:00 p.m.
  • Bus seats are limited.  Limit requested bus pass to one additional bus rider.  Please note that due to COVID-19 FCPS Health Guidelines, bus drivers may refuse additional riders based on capacity.
Reminders: We ask you to communicate with your child(ren) about dismissal plans BEFORE their morning arrival.

This form can only be filled out by the student's legal guardian.

Submission of this form is authorization to release your child to above adult.  
For your child’s safety, a Federal issued ID is required to pick up.
By submitting this form, you are confirming that you are the parent or legal guardian of the student.
This submission will be sent to our main office. A copy of this submission will be sent to the parent/guardian email address which was entered above.
