Important Beginning of the Year Forms

Required Forms

The Health Information Form and the Emergency Care Information Form provide valuable information regarding student health conditions. These forms are critical in the early identification of students who may need support in the educational environment. Along with the development of Healthcare Plans (HCP) and Action Plans, the Public Health Nurses utilize the information contained in the forms to develop teaching strategies for FCPS staff. Even though we are beginning the year virtually, it is critical that all forms are completed. These are available for electronic submission by parents via their ParentVue accounts within the weCare@school (weCare) module. For more information, please visit the FCPS weCare@school site. 

 

A copy of the Emergency Care Information Form was sent home in the Tuesday folder on August 24th.  This form will contain the current information we have on file.  If there are any changes, please update either via the weCare module or bring the updated copy to the school office.  

Emergency Care Card Form 

  • If possible, you can make any changes using weCare@School via your ParentVue account.  We must have an updated form for every student on file at the school. It is extremely important that we have the name and phone numbers of at least two people in the “Other Contact Information” section of the form. Please be sure to notify the school office of any changes in phone numbers or email addresses that occur during the school year by using weCare@School via your FCPS 24/7 Parent View account. 

Health Information Form (Grades 1 - 6 ONLY) 

  • This form needs to be completed annually to inform us about your student’s health condition that affects his or her school day. Please sign and date prior to returning. This form is available for electronic submission by parents via their ParentVue Account within the weCare@school (weCare) module.

Students Rights & Responsibilities Acknowledgement Form

  • Please review the Student Rights and Responsibilities and related resources at www.fcps.edu/srr. After reviewing the information, fill out the SR&R Parent Acknowledgement Form using this link. The form must be filled out one time per student.

Parent/Guardian Consent for Select Digital Resources

  • At Canterbury Woods Elementary School, our teachers will use a variety of digital resources to support student learning and promote communication and collaboration with students and families. Some of the digital resources your child may use this year will require your consent before they can be used. You can find our school’s list of the resources that require parental consent here
  • Included on this page is also a link to a form where the enrolling parent or guardian will need to indicate whether or not their student is allowed to use the list of digital resources this year. This page also provides more information about how FCPS carefully reviews digital resources for approval and why we must request your consent before using certain tools. We encourage you to complete the parental consent for digital resources form as soon as possible, as your child will not be able to use the digital resources until we have received your permission. Please reach out to the school if you have questions or concerns about the digital resources we’ve chosen to use this year.

 Survey of Homeless Students  - This form should be returned to school only if applicable. 

Identification of Military Connected Students Local school divisions are required to identify students who have a parent in the United States uniformed services.

Optional Opt-Out Forms

Annual Notice of Survey, Records, Curriculum, Privacy & Related Rights - Opt- Out Options (complete as applicable)

CPS Annual Notice of Survey, Records, Curriculum, Privacy, and Related Rights and Opt-Out Forms - This booklet provides you with detailed information about your opt-out choices. The form is due no later than the Friday of the 2nd week of school and includes:

  • Objection to Release of Directory Information to the Public
  • School Counseling Opt-Out Forms
  • Objection to Release of Information for Commercial Purposes

Family Life Education Instructional Program Information & Opt-Out Forms

Fairfax County Public Schools (FCPS) provides a comprehensive, sequential Family Life Education (FLE) program for students in grades kindergarten through grade twelve. Instruction is seen as a partnership among parents and guardians, the school, and the community in supporting the learning essential to the development of strong families, positive relationships, and a healthy community.

Parents/guardians may choose to opt their child out of all or part of the Family Life Education program. Students who are opted out are provided with age-appropriate, nonpunitive alternative health instruction, and every effort is made to foster respect for family choices. If you wish to opt your child out of all or part of FLE for this school year, opt out forms have been included in your child’s back-to-school/orientation packets and are available online for Elementary (K-6) here

Family Life Education Opt Out Forms 

Please read and return the opt-out form for your child’s grade level ONLY if you do NOT want your child to participate. Program questions may be addressed at our school by contacting our assistant principal, Mindy Milliron. If you choose to have your child participate in the Family Life Education Program, you do not need to return the form.
